Abstract: Unwarranted variation is a major human factor challenge in health care delivery. This variation is a major source of unintended patient injury in the acute care setting and implementing standardized processes has been shown to be a foremost countermeasure for improving patient safety. However, introducing evidence-based practices is difficult to achieve and sustain. This study reports the development of a high reliability assessment tool and the use of healthcare care bundles as an application of checklist. It is part of a larger three year prospective study of 16 hospitals representing over 110,000 deliveries. The findings indicate that perinatal unit reliability can be measured and that wide variation exists within the nine dimensions of a reliability scale, as well as between hospitals.
