Abstract: Adequate Human Resources for Health (HRH) in terms of quantity and quality are very crucial as they are the frontline of health services. The shortage of health workers in Indonesia particularly serving in very remote area is a critical issue that must be addressed through policy, planning and implementation of innovative ways, such as incentives for retaining and motivating health workers. This paper aims to assess on how an incentive scheme applied based on working location especially on contracted staff. The impact of decentralization system on incentive given to health workers also will be discussed. A desk study was performed from a relevant published materials. The document highlights the importance of financial incentive and non financial incentive. However, financial incentive it self is not enough to retain and motivate health workforce. Incentive system must be integrated within the health system, in line with the goal and objective of Indonesia’s health development. The role of local government under decentralization system also crucial to keep and motivate them in the workplace
