Abstract:

Patients/Clients are not monologues and different nursing care is required for different clients. Nursing practice which is more focused on providing care to clients on site have been practiced over decades but due to the increase in expectations from the clients, a more complete of individualized care is expected from nurses. Case management is proven a complete care to client which demonstrate a significant improvement in quality of care, increase quality of life, increase client’s satisfaction, decrease healthcare expense, reduce admission rate and readmission rate, Nevertheless, there is a limitation of applying case management care to every client in terms of increasing nurses’ workload, higher requirement of nurses. Health care system also hinders the using of case management due to the shortage of nurses, limited resources and time consuming. Therefore, emphasis should be placed on maintaining sustainable case load for the case managers, training professional case managers, integration of health and social care system, establish mechanism on pool budget and lastly nurses should share the same vision and objectives on the application of case management care.
