Effective Ways to Prevent Readmission for Patients with Chronic Congestive Heart Failure: A Preliminary Critique of the Literature

Abstract: Chronic Congestive Heart Failure (CHF) presents a dilemma for patients and facilities. A preliminary literature review to explore maintenance back in the community after discharge to prevent readmission within 30days of discharge was instituted. Naylor’s Transitinal Care Model was utilized as the theoretical framework for this foray into the literature and possible interventions for the APRN.
