Abstract:
Continuous long-term treatment of schizophrenia with antipsychotic drugs needs effective management for symptoms control. It is still lack of guidance for drug management in nonpsychiatric hospital settings. The purpose of this study was to develop and assess the pharmacy mental status examination (PMSE) for Thai pharmacists to monitor the clinical response in schizophrenic patients. Methods: Three psychiatrists and two pharmacists were participated in the study. Forty-eight schizophrenic patients from Suan Prung Psychiatric Hospital, Chiang Mai, Thailand were enrolled in the study between July-August, 2015. The mental status examination Thai version (MSE-T) was modified for the PMSE. The reliability and validity were assessed. Results: The 4 items from MSE-T including consciousness, attention & concentration, memory, and judgment were prior excluded because of more specific skills needed. All items were resulted in good content validity (IOC = 0.67-1.00). The internal consistency of the PMSE was very good (Cronbach’s alpha = 0.853). Concurrent validity was a strong, positive correlation between severity result of the PMSE and the MSE-T, which was statistically significant (rs (34) = 0.963, p < 0.001). The inter-rater reliability for test of agreement, expressed as Kw, between one psychiatrist and one pharmacist using the PMSE at the same time = 0.976 (p < 0.001). Conclusion: The PMSE appears to be a valid tool for Thai pharmacists to monitor the clinical response of antipsychotic drug treatment in patients with schizophrenia.
