Abstract: Personal Health Records (PHR) are one of the technologies that have emerged in order to move health care from a passive perspective to an active one. Despite these tools are really useful, they can be improved developing tools that allow clinicians and patients to better understand and act on the information. For this reason, this paper presents two approaches that can help to achieve this understanding, all of them based on the use of ontologies and standardized medical vocabularies.
