Initiation of hemodialysis is the process of commencement of hemodialysis as a renal replacement therapy performed on chronic kidney patients. In interviews authors with patients who underwent HD, one in five patients who experienced CKD often refused to do dialysis. Some of the reasons are dialysis speed up the patient's death, making the patient ugly because of blackening, and the procedure is painful. But there has never been any research on this HD initiation. The purpose of this study was to get the reasons for rejection or initiation of hemodialysis. The design of this research was qualitative research using phenomenological study approach. The research conducted on July 28-August 28, 2016 in Prof. Dr. W.Z Johannes Kupang hospital. Data analysis using Colaizzi approach. The results of the study found six themes in participants who rejected HD: Lack of medical expenses, Worrying that no one will accompany the patient during HD, A long distance between home and place of HD, HD creates fear, stress, and expectation of death, Patient’s preference to traditional medicine. The participants who want to initiate HD have three themes that believing doctor’s preference to traditional medicine, witnessing one’s own family and encouragement from another family member with the history of HD.
